So that we may serve you quickly, we ask that you arrive at least 10 minutes early for your appointment so we can handle any necessary paperwork.

Current patients

Please be sure to bring:

• Your insurance card

• Identification (e.g. driver's license)

• Payment for your copay

New patients

If this is your first appointment with us, please review the Patient Forms below and complete as much as possible. You should be able to fill in these forms on your computer and then print them out and bring them to your appointment (for privacy reasons please don't email your forms).

Please also bring:

• A list of medicines you are taking
